455 A Case of An Unusual Penetrating Brain Injury

Abstract Introduction Penetrating brain injuries (PBI) are less common than closed head traumas. Interest in frontal lobe dates back to 1848’s famous Phineas Gage incident. Here, we report a case of construction worker, who showed remarkable neuropsychiatric outcome following workplace accident. Case Presentation 45year old gentleman came with complaint foreign body accidentally lodged his forehead. He had no neurological deficits or CSF leak. X-ray revealed that 5.5cm metallic screw was penetrating skull, headfirst; CT approximately 3cm the sinus. given antiepileptics and antibiotics prophylactically. Surgically removed by raising bone flap mobilizing carefully cutting dura adequate irrigation, causing minimal damage parenchyma. The dural were repaired sinus canalized, then drain placed, patient shifted ICU for postoperative monitoring. Discussion It is important prognosticate PBI as they can have may be lifelong. Understanding mechanism injury, aggressive medical management immediate surgical intervention lead improved outcomes. A object size 5.5cm, completely skull would otherwise resulted extensive parenchymal damage. Literature suggests two people an identical In our has served purpose protection against injury (of only about 1cm).
